In accordance with the Medfield Youth Basketball Association Safekids program all prospective coaches need to submit information to request a CORI report from the state of Massachusetts.

Please complete the CORI form below. Having your CORI processed is a requirement for coaching.

The data is stored securely in the MYBA Google account and is only accessible by select members of the Board. PII data will be deleted once all CORI forms have been processed.
