In accordance with the Medfield Youth Basketball Association Safekids program all prospective coaches need to submit information to request a CORI report from the state of Massachusetts.

Please download and complete the form and mail/scan it along with a copy of your valid Massachusetts license or other authorized state ID to  Dave Higgins.
